Power Science Engineering LLC performs field evaluations in accordance with NFPA 790 and 791 and the National Electric Code (NEC). Upon completion of the field evaluation, if the equipment conforms to the applicable standards, a statement of conformity will be issued for the equipment included in the scope of work on our proposal. Our statement of conformity will not apply to identical or similar units that were not directly evaluated and labeled. The statement of conformity that we issue will only apply to the equipment where, and as, it is installed at the time of the evaluation. Any modification or relocation of the equipment after the statement of conformity is issued will nullify the statement and label and will require another evaluation. By filling out and submitting this form you acknowledge that you are applying for field evaluation services on behalf of the entity listed above and are an authorized representative. With this application please submit all available technical information and electrical drawings. We shall implement strict confidentiality measures when reviewing and handling documents. Upon review of this and any supplied documents Power Science Engineering LLC will respond by providing a proposal outlining costs or with a statement of refusal. Upon receipt and acceptance of a proposal it will be the responsibility of the applicant to provide safe access to all equipment identified in the scope of work and to power down and lock-out any equipment to be evaluated. This application does not create any financial or contractual obligation other than acknowledging that you are aware of what a field evaluation entails and are interested in field evaluation services and are willing to provide the information required to generate a proposal.
